Abstract

Аутоиммунный гепатит АИГ представляет собой прогрессирующее хроническое заболевание печени, неизвестной этиологии, с разнообразной клинической картиной и эволюцией. Диагноз аутоиммунного гепатита ставится на основании гистологических изменений перипортальный гепатит, характерных клинических и биологических признаках гипертрансаминаземия, повышение концентрации сывороточного IgG или гипергаммаглобулинемия, при наличии в сыворотке широкого спектра аутоантител. Цель данной статьи состояла в том, чтобы проанализировать эволюцию заболевания пациентки с аутоиммунным гепатитом во время беременности, безопасность лечения и отдаленные результаты/ осложнения иммуносупрессивной терапии. В этот период пациентка периодически игнорировала лечение, потеряла одну беременность, а затем сумела родить здорового ребенка. Преднизолон в качестве начальной терапии, с последующим добавлением азатиоприна через две недели, является терапией первой линии при АИГ, в том числе при беременности. Для данной пациентки иммуносупрессивная терапия преднизолоном и азатиоприном была безопасной и эффективной, обеспечивая индукцию и поддержание ремиссии, без значительных осложнений или обострений во время беременности. Таким образом, точный установленный диагноз и своевременное применение соответствующего лечения, строгий контроль за течением заболевания и осложнениями иммуносупрессивной терапии, могут способствовать благоприятному течению АИГ и замедлению прогрессирования заболевания.

Autoimmune hepatitis AIH is a progressive chronic liver disease of unknown cause, with varied clinical picture and evolution. The diagnosis of autoimmune hepatitis is based on histological changes interface hepatitis, characteristic clinical and biological signs hypertransaminasemia and increased serum IgG concentration, with the presence of one or more characteristic autoantibodies. The purpose of this presentation was to analyze the evolution of the disease of a patient with autoimmune hepatitis during pregnancy, the safety of the treatment, and the long-term results/complications of immunosuppressive therapy. During this period, the patient periodically stopped the treatment, had a pregnancy that ended with the intrauterine death of the fetus, and then managed to give birth to a healthy child. Prednisolon as initial therapy, followed by the addition of azathioprine after two weeks, are the first-line treatment for AIH, including in pregnancy. In our patient, immunosuppressive therapy with prednisone and azathioprine was safe and effective in inducing and maintaining remission, without significant complications or exacerbation during the pregnancy. Thus, an early established diagnosis and the prompt application of appropriate treatment, strict monitoring of the disease and of immunosuppressive treatment complications, can improve the evolution of AIH, slowing the progression of the disease.

Hepatita autoimună HAI este o boală hepatică cronică progresivă, de cauză neidentificată, cu tablou clinic și evoluție variate. Diagnosticul de hepatită autoimună se bazează pe modificările histologice hepatită de interfață, semnele clinice și biologice caracteristice hipertransaminazemie și creșterea concentrației serice de IgG, cu prezența unuia sau mai multor autoanticorpi specifici. Scopul acestui studiu a fost analiza evoluției bolii unei paciente cu hepatită autoimună în sarcină, siguranța tratamentului, cât și rezultatele/ complicațiile pe termen lung la terapia imunosupresoare. În această perioadă pacienta a ignorat tratamentul periodic, a pierdut o sarcină, apoi a reușit să nască un copil sănătos. Terapia inițială cu prednisolon, urmată de adăugarea azatioprinei după două săptămâni, este tratamentul de primă linie pentru HAI, inclusiv în sarcină. La pacienta prezentată, terapia imunosupresoare cu prednison și azatioprină a fost sigură și eficientă în inducerea și menținerea remisiunii, fără complicații semnificative sau exacerbare pe durata sarcinii. Astfel, un diagnostic stabilit precoce si aplicarea promptă a tratamentului oportun, monitorizarea riguroasă a bolii și complicațiilor tratamentului imunosupresiv, pot favoriza evoluția HAI, încetinind progresia bolii autoimune.
