
Apendicita acută la bătrâni a fost considerată o boală rară, ulterior s-a observat că incidența apendicitei acute cu vârsta nu scade. Formele distructive de AA sunt detectate la vârstnici mai frecvent decât la tineri 60,7 -73% din cazuri, dintre care la 36% de pacienți se constată AA apendicita acută perforativă. De la prima descriere a AA în 1839, de Bright și Addison, pînă în prezent, principala metodă de diagnostic a AA a rămas a fi cea clinică, care se bazează pe colectarea anamnezei și determinarea semnelor locale ale bolii. Scorurile diagnostice aplicate în diagnosticul AA sunt foarte diferite atât prin indicii și manifestările clinice abordate în scoruri cât și prin pontajul atribuit și interpretare. Putem menționa cu certitudine că este o necesitate studierea aspectelor organizatorice și juridice de utilizare SD scorurilor de diagnostic în AA în practica chirurgicală curentă, realizarea unei formulări fundamentate științific a principiilor metodologice unificate pentru crearea și evaluarea comparativă a acestora, scopul fiind elaborarea unui scor diagnostic clinico-ecografic al AA, țintit pe diagnosticul formelor distructive ale AA.
Острый аппендицит у пожилых считался редким заболеванием, позже было замечено, что заболеваемость острым аппендицитом с возрастом не снижается. Деструктивные формы ОА острoго аппендицита выявляют у лиц пожилого возраста чаще, чем у лиц молодого возраста 60,7-73% случаев, из них у 36% больных перфоративная ОА. Со времени первого описания АА в 1839 г. Брайтом и Аддисоном и до настоящего времени основным методом диагностики ОА остается клинический, основанный на сборе анамнеза и определении местных признаков заболевания. Диагностические шкалы, применяемые при диагностике ОА, сильно различаются как по показаниям, так и по клиническим проявлениям, которые учитываются в шкалах, а также по присваиваемой шкале и интерпретации. С уверенностью можно отметить необходимость изучения организационно-правовых аспектов применения ДШ диагностических шкал при ОА в современной хирургической практике, достижения научно-обоснованной формулировки единых методологических принципов их создания и сравнительной оценки с целью разработки клинико-ультразвуковой диагностической шкалы ОA, направленной на диагностику деструктивных форм ОA.
Acute appendicitis in the elderly was considered a rare disease, later it was observed that the incidence of acute appendicitis does not decrease with age. Destructive forms of AA acute appendicitis are detected in the elderly more frequently than in young people 60.7-73% of cases, of which 36% of patients have perforative AA. The diagnostic scores applied in the diagnosis of AA are very different both in the indications and clinical manifestations addressed in the scores and in the scoring assigned and interpretation.
