Abstract

ÖZET İnce dişeti fenotipinin, dişeti çekilmeleri için önemli bir risk faktörü olduğu bilinmektedir. Ortodontik tedavi sırasında ve sonrasında ince dişeti fenotipi olan hastalarda, daha fazla dişeti çekilmesi görüldüğü bir gerçektir. Özellikle kemik dehisensleri ve fenestrasyonları olan, dişlerin vestibüldekonumlandığı bölgelerde dişeti beslenemediği için dişeti çekilmesi meydana gelmektedir. Dişeti çekilmelerinin tedavisinde kullanılan cerrahi yöntemlerde, çoğunlukla avasküler greftlerin dişeti çekilmesibölgesine yerleştirilmesi söz konusudur. Kanlanması sınırlı bir alana cerrahi uygulamalarla, bölgenindamarsal desteğini azaltarak ve avasküler greftler uygulayarak yapılan tedavilerin önemli limitasyonlarıgöze çarpmaktadır. Son yıllarda geleneksel tedavi yöntemlerine yeni teknikler ve yeni greft materyallerieklenerek, bu teknikler kullanılmaya başlanmıştır. Dişeti çekilmesi veya ince dişeti fenotipi olan bölgelere, trombositten zengin fibrinler (PRF); minimal invaziv yöntemler kullanılarak veya cerrahi olmadanuygulanabilmektedir. Tamamen otojen ve yoğunlaştırılmış kan pıhtıları olan bu kan konsantrelerinin,dişeti çekilmeleri için uygulanan geleneksel tedavi yöntemlerinin limitasyonlarını azaltması beklenmektedir.Anah tar Ke li me ler: Diş eti; fenotip; yara iyileşmesi; kan trombositleri; rejenerasyonABS TRACT Thin gingival phenotype is known as an important risk factor for gingival recession. Moregingival recessions are observed in patients with thin gingiva during and after orthodontic treatment.Gingival recession occurs where the teeth are positioned in the vestibule, especially areas with bone dehiscences and fenestrations, due to insufficient gingival vascular supply.Today, the methods used forthe treatment of the gingival recessions are surgical methods, and these methods mostly include theplacement of avascular grafts at the gingival recession area. Significant limitations of the treatments,which are performed surgically, are noticeable when the application of avascular grafts to the areas withreduced vascular supply. In recent years, some new techniques and new graft materials are added to traditional treatment methods. Platelet-rich fibrins (PRFs) can be applied to the areas with gingival recessions or thin gingival phenotype by minimally invasive surgical methods or non-surgically. These bloodconcentrates, which are completely autogenous and concentrated blood clots, can be expected to reducethe limitations of traditional treatment methods for gingival recessions.Keywords: Gingiva; phenotype; wound healing; blood platelets; regeneration
