Abstract

Колоректальный рак продолжает занимать одно из первых мест в структуре онкологической заболеваемости и смертности как в краевом, так и в общероссийском регистре. Единственным эффективным методом профилактики развития колоректального рака и снижения смертности от этой патологии является решение проблемы скрининга и своевременного радикального лечения. Эндоскопическая диагностика новообразований толстой кишки на сегодняшний день является наиболее информативной методикой скрининга, в том числе обладая возможностью проведения вторичной профилактики. Впервые в России под эгидой Российского общества эндоскопии (РОЭНДО) было проведено крупное рандомизированное исследование на основе международного проекта «QUACOL», целью которого было определение индикации оценки качества выполнения колоноскопии. В статье проведен собственный анализ выполнения 3484 колоноскопий в рамках участия в этом проекте. Доля выполнения тотальных колоноскопий в нашем центре составила 89,3%. Частота выявления полиповидных образований толстой кишки составила 36,3%, доля обнаруженных аденом с дисплазией низкой степени – 22,9%, а «запущенных» аденом – 7,6%. Полученные результаты позволили указать на перспективы для улучшения качества эндоскопической диагностики и лечения патологии толстой кишки.

Colorectal cancer occupies one of the leading positions among oncological diseases, both in the regional and all Russian register. The only effective method to decrease mortality rate is proper screening and timely radical treatment. Endoscopic diagnosis for colonic malignancies is the most accurate and informative option. We have performed a major randomized assessment together with Russian Endoscopic Society (ROENDO) to define quality of colonoscopy. In the present paper we presented our analysis for 3 484 colonoscopies. In our center we performed about 89.3% of all procedures. Polyp neoplasms rate was 36,3%, adenomas with low grade of dysplasia rate was 22,9%, and a rate of advanced adenomas was 7,6%. These results showed further prospectives for improving quality of endoscopic diagnosing and management of colonic pathology.
