Abstract

Сравнивали диагностические возможности гидроэхоколонографии (ГЭК) и ирригографии (ИГ) в выявлении признаков болезни Гиршспрунга (БГ), а именно переходной зоны (ПЗ), у 65 детей с запором в возрасте от 2 дней до 15 лет. Чувствительность ГЭК и ИГ составила 75 и 67,9%, специфичность 91,9 и 94,6% соответственно. Более высокая чувствительность ГЭК объясняется тем, что при проведении эхографии присутствуют дополнительные факторы, способствующие обнаружению ПЗ: возможна оценка перистальтики одновременно с визуализацией просвета кишки и структуры кишечной стенки. ГЭК информативный, не несущий лучевой нагрузки метод исследования толстой кишки у детей, демонстрирующий высокую чувствительность и специфичность в выявлении ПЗ. ИГ может быть исключена как лишняя ступень в диагностике БГ, так как она используется исключительно как скрининговое исследование в идентификации ПЗ, но окончательный диагноз подтверждается гистологическим и гистохимическим методами.

The study was designed to compare effectiveness of hydroechocolonography (HEC) and irrigography (IG) for the detection of signs of Hirschsprung’s disease (HD) in 65 children aged 2 days -15 years suffering constipation with special reference to the transition zone (TZ). Sensitivity of HEC and IG was estimated at 75 and 67.9%, specificity 91.9 and 94.6% respectively. The higher sensitivity ofHEC is due to the presence of additional factors during echography facilitating detection of HD. They make possible evaluation of peristalsis simultaneously with visualization of the intestinal lumen and wall structure. HEC is an informative radiation-free method for the study of colon in children allowing to reveal TZ with high sensitivity and specificity. IG is an unnecessary procedure in diagnostics of HD. It can be used only as a tool for TZ screening, but the definitive diagnosis needs to be confirmed by histological and histochemical methods.
