Abstract

Проведен анализ эффективности применения различных способов мозаичной хондропластики дефектов гиалинового хряща при деструктивно-дистрофических и посттравматических поражениях коленного сустава. На основании серии экспериментальных исследований предложены способы мозаичной хондропластики, заключающиеся в применении костных аутотрансплантатов без пропитывания и с пропитыванием взвесью культивированных in vitro аутогенных хондроцитов. Оценка эффективности разработанных способов хондропластики была проведена на основании принципов доказательной медицины – с помощью построения математической модели функционального состояния нижней конечности и проведения системного многофакторного анализа динамики функционального восстановления коленного сустава. Клиническая оценка основана на определении показателей WOMAC-индекса, индекса Лекена, оценке статико-динамических параметров опорно-двигательной системы с помощью подометрии, гониометрии, функциональной электромиографии, стабилометрии. Анализ полученных данных показал, что использование аутохондропластики обеспечило достоверно более выраженное структурно-функциональное восстановление коленного сустава и применение клеточных технологий в замещении дефектов гиалинового хряща наиболее перспективно.

The analysis of the effectiveness of different methods of mosaicplasty hyaline cartilage defects in the destructive-degenerative and traumatic lesions of the knee joint. Based on a series of experimental studies proposed methods chondroplasty mosaic based on the use of bone grafts without soaking and soaking autologous chondrocytes. Evaluating the effectiveness of the developed methods chondroplasty was carried out on the basis of the principles of evidence-based medicine, a mathematical model of the system and multivariate analysis of the dynamics of the functional recovery of the knee. Clinical evaluation is based on the definition of indicators WOMAC-index, Lequesne index, assessing static and dynamic parameters of the musculoskeletal system through basograph, goniometry, functional electromyography stabilometry. Analysis of the data showed that the use of autochondroplasty provided significantly greater structural and functional recovery of the knee and the use of cellular technology in the replacement of hyaline cartilage defects of the most promising.
