Трансанальная эндоскопическая Микрохирургия в лечении опухолей прямой кишки

Abstract

Проведен анализ результатов лечения 46 больных, оперированных методом трансанальной эндоскопической хирургии по поводу аденом и начальных форм рака прямой кишки. Полностенному удалению подвергались полипы прямой кишки (65%), удаление опухоли вместе с подлежащей мезоректальной клетчаткой осуществлялось при полипах с признаками малигнизации, начальном раке прямой кишки (35%). Послеоперационное патоморфологическое исследование у 15% больных в аденомах прямой кишки выявило участки аденокарциномы in situ. Послеоперационные осложнения развились у 3 больных (6,5%). За время наблюдения рецидив опухоли прямой кишки выявлен у 2 больных (4,3%) через 10-14 месяцев после операции. Трансанальное эндохирургическое вмешательство на прямой кишке не оказало существенного влияния на функцию прямой кишки. Описанные возможности позволяют рекомендовать трансанальное эндоскопическое удаление опухолей как метод выбора у больных начальными формами рака прямой кишки.

The results of transanal endoscopic microsurgery in 46 patients with adenomas and early stage rectal cancer were analyzed. Patients with rectal polyps (65%) received total wall excision, and patients with polyps with malignization signs and early stage rectal cancer (35%) received mesorectal excision. Postoperative pathomorphological examination showed areas of adenocarcinoma in situ in rectal adenomas in 15% of patients. 3 patients (6.5%) developed postoperative complications. Recurrent rectal tumor was observed in 2 patients (4.3%) 10-14 months after the surgery. Transanal endoscopic microsurgery did not affect the rectum function. The possibilities described allow recommending transanal endoscopic microsurgery as a method of choice for patients with early stage rectal cancer.
