Анальгезия родов у женщин, страдающих сахарным диабетом

Abstract

The article deals with a study of duration ofphysiological labors in women with diabetes mellitus. Patients and methods: 120 women were devided into three groups according to method of analgesia. We assessed data of central haemodynamics, efficacy of analgesia, glucose level in blood, conditions offetus and newborn. Results: We found that epidural analgesia with bupivacaine hydrochloride was an optimal method of labor analgesia in women with diabetes mellitus. This method provided a stabilization of glucose level in blood, normalization of data of central haemodynamics and allowed to avoid anomalies of labor. Conclusions: Analgesia with promedol and paracetamol is a preferable method in case ofpresence of epidural analgesia contraindications.

Изучено течение физиологических родов у 120 женщин, страдающих сахарным диабетом. В зависимости от метода обезболивания родов роженицы были разделены на 3 группы. В процессе родов оценивали параметры центральной гемодинамики, эффективность анальгезии, уровень глюкозы крови, состояние плода и новорожденного. Установлено, что оптимальным методом обезболивания при самопроизвольных родах у женщин, больных сахарным диабетом, является эпидуральная анальгезия ропивакаина гидрохлоридом, обеспечивающая стабилизацию уровня глюкозы в крови, нормализацию параметров центральной гемодинамики и купирование аномалий родовой деятельности. Анальгезия промедолом и парацетамолом предпочтительна при наличии противопоказаний к эпидуральной анальгезии.
