Abstract

Изучены результаты лечения 333 больных с кишечной непроходимостью, которым были выполнены резекции тонкой кишки c первичными анастомозами. Несостоятельность анастомозов развилась в 34 случаях (10,2%), послеоперационная летальность составила 16,5% (умерло 55 больных). При анализе выживаемости больных с тонкокишечной непроходимостью, которым проведена резекция тонкой кишки, установлено, что достоверное влияние оказывает возраст пациентов, тяжесть состояния, длительность предоперационного периода, тип анастомоза, объем резекции приводящего отдела, осложнения со стороны анастомоза и системные осложнения. Увеличение объема резекции жизнеспособного приводящего отдела тонкой кишки приводит к достоверному снижению летальности, снижению уровня эндоинтоксикации, снижению количества абдоминальных и системных осложнений. Оптимальными анастомозами являются тонкокишечные «конец в конец» и инвагинационные тонко-толстокишечные «конец в бок».

The results of surgical treatment of 333 patients with the intestinal obstruction, who underwent the small intestine resections with primary anastomoses were studied. There were complications which took place in 34 (10, 2%) cases after the anastomoses. Lethal outcomes were in 55 (20,5%) cases. It has been determinated that survival rate of the patients after small intestinal resection is influenced by their age, clinical condition, lasting of preoperative period, type of anastomosis, volume of the adducent region resection, local and systemic complications. It has been established that extension of the resection of viability adducent small intestinal region results in mortality rate decrease, intoxication decrease and decrease of the abdominal and systemic complications It has been determined that optimal anastomoses are «end to end» of the small intestine and «end to side» between the small and the large intestine.
