Возможности прямой реваскуляризации конечности в лечении пациентов с синдромом диабетической стопы

Abstract

The review of researches dedicated to studying of the possibilities of the lower limbs revascularization performance at treatment of patients with diabetic foot syndrome is presented in the article. Application of the revascularization methods is noticed to be an important instrument providing the risk decrease of the lower limbs amputation for the patients with diabetes mellitus. The approaches to usage of open and endovascular methods of revascularization at critical lower limbs ischemia in patients with diabetic foot syndrome are studied. Progressing of new technical possibilities of the vascular surgery is responsible for reorientation to minimal invasive treatment mainly due to X-ray angiographic variants of revascularization. The conducted review of the researches demonstrates the following: indications for definite techniques of revascularization haven,t been worked out depending on stenosis localization, its extent, severity degree of diabetic foot syndrome, functional state of outflow and inflow ways; that demands further working out of the given problems.

В статье представлен обзор исследований, посвященных изучению возможностей проведения реваскуляризации нижних конечностей при лечении пациентов с синдромом диабетической стопы. Отмечено, что применение методов реваскуляризации является важным инструментом, обеспечивающим снижение риска ампутации нижних конечностей у пациентов с сахарным диабетом. Рассмотрены подходы к использованию открытых и эндоваскулярных методов реваскуляризации при критической ишемии нижних конечностей у пациентов с синдромом диабетической стопы. Развитие новых технических возможностей сосудистой хирургии обусловливает переориентацию на малоинвазивные методы лечения преимущественно за счет рентгеноангиографических вариантов реваскуляризации. Проведенный обзор исследований демонстрирует: показания к применению отдельных методик реваскуляризации не отработаны в зависимости от локализации стеноза, его протяженности, степени тяжести синдрома диабетической стопы, функционального состояния путей притока и оттока, что требует дальнейшей проработки данных вопросов.
