Abstract

The paper presents an analysis of the 32 observations of recurrent diffuse toxic goiter and multinodular. The median duration of clinical signs of recurrent goiter ranged 16 (6-27 years). When toxic goiter volume fraction of thyroid ranged from 1 to 138 ml, eutireoidnom from 3 to 413 ml. Before surgery, 2 patients revealed bilateral paresis of the larynx, 1 laryngeal hemiparesis. The incidence of laryngeal nerve dysfunction after surgery performed earlier was 7.8 %, postoperative hypoparathyroidism 3.2 %. Extirpation of the thyroid stump in recurrent toxic goiter performed 20 patients, including two-way 18. Extirpation of the stump in recurrent eutireoid goiter was performed in 12 patients, including 6 cases of bilateral extirpation. The frequency of postoperative paresis of the larynx in terms of more than 6 months after surgery was 6 % (for each nerve at risk of injury), after initial thyroidectomy 0 %. Quality of life, estimated on a scale of the SF-36 in terms of more than 6 months are statistically significantly lower on a scale that PF (p = 0,049), than in the primary surgical intervention. The analysis shows the benefit of the implementation of primary thyroidectomy for diffuse toxic goiter and multinodular

В статье представлен анализ 32 наблюдений рецидивного диффузного токсического и многоузлового зоба. Медиана длительности клинических проявлений рецидивного зоба составила 16 (6-27 лет). При токсическом зобе объем долей щитовидной железы варьировал от 1 до 138 мл, при эутиреоидном от 3 до 413 мл. До операции у 2 больных выявлен двухсторонний парез гортани, у 1 односторонний парез гортани. Частота выявления нарушения функции гортанных нервов после выполненной ранее операции составила 7,8 %, послеоперационного гипопаратиреоза 3,2 %. Экстирпация культи щитовидной железы при рецидивном токсическом зобе выполнена 20 больным, в том числе двухсторонняя 18. Экстирпация культи при рецидивном эутиреоидном зобе выполнена 12 больным, в том числе двухсторонняя 6. Частота послеоперационного пареза гортани в сроки более 6 месяцев после операции составила 6 % (на каждый нерв, подверженный риску повреждения), после первичной тиреоидэктомии 0 %. Качество жизни, оцененное по шкале SF-36 в сроки более 6 месяцев статистически достоверно ниже по шкале PF что (p = 0,049), чем в группе первичной тиреоидэктомии. Результаты анализа свидетельствуют в пользу выполнения первичной тиреоидэктомии при диффузном токсическом и многоузловом зобе.
