Abstract

Цель – разработать обоснованные с позиций нормальной и патологической физиологии методы диагностики нарушений оксигенации и торакопульмональной рестрикции во время респираторной поддержки и оценить их эффективность. Обследовано 206 пациентов, получающих длительную респираторную поддержку. Разработан способ диагностики нарушений оксигенации в процессе искусственной вентиляции легких, позволяющий в 1,4 раза чаще диагностировать нарушение оксигенации, чем по шкале повреждения легких Мюррея. Разработан способ диагностики торакопульмональной рестрикции, имеющий в 1,9 раза большую чувствительность и в 4,4 раза большую специфичность, чем шкала повреждения легких Мюррея. Предложены новая модификация шкалы острого повреждения легких и калькулятор функции внешнего дыхания.

The goal of the study was to develop methods for diagnosing oxygenation disorders and thoracopulmonary restriction during respiratory support and to evaluate their efficacy. 206 patients receiving prolonged respiratory support were included in the study. The developed method of diagnosing oxygenation disorders during respiratory support is 1.4 times faster than the Murray’s lung ingury score. Sensitivity and specificity oftheauthors’method fordiagnosing thoracopulmonaryrestrictionamounted to0.897and 0.838 respectively as compared to 0.47 and 0.189 achieved by using the Murray’s lung injury score. A new modification of the acute lung injury score including a respiratory function calculator was also developed.
