Динамическая регуляция церебрального кровотока и ее клинико-лабораторные маркеры у пациентов пожилого возраста с артериальной гипертензией

Abstract

Цель: оценить состояние динамической регуляции церебрального кровотока (ЦК) у пациентов пожилого возраста с артериальной гипертензией (АГ) и возможность использования клинико-лабораторных параметров в качестве его маркеров. Материал. Обследовано 179 пациентов позднего возраста с АГ. Оценка параметров ЦК проводилась на ультразвуковой системе Philips Envisor HD (USA) в процессе ортостатической пробы (ОРП). Результаты. Изменения параметров ЦК в процессе ОРП у лиц пожилого возраста достигали степени статистической достоверности (Friedman ANOVA р 0,0091... 0,012, коэф. конкордации 0,308... 0,691). Выявлены тендерные различия динамики параметров ЦК в процессе ОРП, а также особенности динамики регуляции ЦК в процессе ОРП у больных с медикаментозно корригированным артериальным давлением (АД) и больных, не достигших целевого уровня АД. Заключение. Пол, достижение целевых уровней АД, цитоморфологические параметры периферической крови, прием диуретиков могут рассматриваться как клинико-лабораторные маркеры состояния динамической ауторегуляции церебрального кровотока у лиц пожилого возраста с АГ

Aim: to estimate the dynamic regulation of cerebral blood flow (CBF) and significance of clinical, and laboratory parameters as a possible markers of CBF violations. Material. 179 elderly patients with arterial hypertension were involved in the study. Transcranial Doppler ultrasonography (Philips Envisor HD, USA) was used for CBF parameters dynamics during orthostatic hypotension. Results. Main directions of CBF parameters changings (Friedman ANOVA p 0,0091... 0,012, coeff. of concordance 0,308... 0,691) were detected. Gender differences of CBF parameters, as well as blood pressure level influence upon CBF regulation during orthostatic hypertension were studied. Conclusion. Received data allow us to consider gender, controlled or uncontrolled hypertension, cytomorphological parameters of peripheral blood, diuretics usage as clinical, and laboratory markers of dynamic regulation of CBF in elderly patients with arterial hypertension
