Ревматоидный артрит: влияние болезни на социальные аспекты качества жизни

Abstract

Цель. Оценить влияние ревматоидного артрита (РА) на социальные аспекты качества жизни пациентов. Материал и методы. Исследованы 104 пациентки, сходные по возрасту, из них 45 с РА (основная группа), 30 с остеоартозом ОА (1-й контроль), 29 с невротическими расстройствами HP (2-й контроль). С помощью Шкалы оценки нарушений и затруднений определена степень неблагоприятного влияния болезни в сферах профессиональной, общественной и семейной жизни. Результаты. Пациентки с РА оценивали влияние болезни на социальные аспекты жизни как более разрушительное, чем больные с ОА, особенно в семейной и общественной сфере. При этом выявлено сближение групп больных РА и HP по суммарно более высокой оценке неблагоприятного влияния болезни на социальное функционирование в целом и преимущественному повреждению сферы семейной жизни. Заключение. Полученные данные свидетельствуют о более высокой психосоциальной уязвимости больных РА в сравнении с больными ОА. Предполагается сходная значимость психических механизмов, формирующих низкую самооценку и затруднения социального приспособления у больных РА и HP. Результаты исследования актуализируют разработку программ психосоциальной реабилитации для больных РА.

Objective. To study rheumatoid arthritis (RA) influence on pts quality oflife social aspects. Material and methods. 104 pts of comparable age were examined. 45 of them had RA, 30 osteoarthritis (OA) and 29 neurotic disturbances (ND). Disease adverse influence degree in professional and social fields and in family life was assessed with disorders and difficulties assessment scale. Results. RA pts considered disease influence on social aspects of life as more destructive than OA pts particularly in family and social life. RA and ND groups were similar in totally higher assessment of unfavorable disease influence on social functioning in the whole and predominant impairment of family life. Conclusion. The present data show higher psychosocial vulnerability of RA pts in comparison to OA pts. Similar significance of psychic mechanisms forming low self-appraisal and difficulties of social adaptation in RA and ND pts is presumed. Study results prove actuality of psychosocial rehabilitation programs development for RA pts.
