Abstract

Arterial Hypertension is one of the main problems of the modern medicine. The most serious and the most common complications are hypertonic encephalopathy and chronic heart failure. In clinical picture of hyper-tonic encephalopathy the essential place is taken with the symptoms showing the presence of arterial and venous cerebral dysfunction and chronic heart failure. We have examined 103 patients with various stages of hypertonic encephalopathy. Most of the patients (89%) have chronic heart failure, severity of which grew, with the progres-sion of hypertonic encephalopathy. Ultrasonic research of main head arteries and brain vessels (arteries and veins) has shown, that on a measure of the diseases progress there appear the phenomena showing structural changes in a wessel wall, disorder of autoregulation mechanismus and dysfunctions of brain arterial and venous system. Treatment of patients with atrial hypertension should consider the possible remodeling all target orgsns and especially the heart.

Артериальная гипертония (АГ) является одной из наиболее актуальных проблем современной медици-ны. Серьёзными и самыми распространенными осложнениями АГ являются гипертоническая энцефа-лопатия (ГЭ) и хроническая сердечная недостаточность (ХСН) В клинической картине больных ГЭ су-щественное место занимают симптомы, свидетельствующие о наличии артериальной и венозной цереб-ральной дисфункции и ХСН. Обследовано 103 больных с различными стадиями ГЭ. У большинства больных (89%) выявлена ХСН, тяжесть которой нарастала по мере прогрессирования ГЭ. Ультразвуко-вое исследование магистральных артерий головы и мозговых сосудов (артерий и вен) показало, что по мере прогрессирования ГЭ и ХСН нарастают явления, свидетельствующие о структурных изменениях в стенке сосудов, нарушении механизмов ауторегуляции и дисфункции артериальной и венозной системы мозга. Лечение пациента с АГ должно учитывать возможное ремоделирование всех органов-мишеней АГ и, в первую очередь, сердца.
