Сравнительная оценка продленной блокады бедренного нерва и эпидуральной анальгезии для послеоперационного обезболивания тотального эндопротезирования коленного сустава

Abstract

В исследовании сравнили 2 наиболее часто обсуждаемых метода послеоперационного обезболивания тотального эндопротезирования коленного сустава. Операцию проводили в условиях субарахноидальной анестезии с внутривенной седацией. 9 пациентам 1-й группы в составе мультимодальной анальгезии проводили продленную блокаду бедренного нерва, 8 пациентам 2-й группы эпидуральную инфузию наропина. При появлении болевых ощущений выполняли болюс раствора местного анестетика. В качестве "анальгезии спасения" при неэффективности основного метода обезболивания дополнительно внутримышечно вводили 20 мг промедола. У пациентов 2-й группы субъективная оценка послеоперационного обезболивания по шкале ВАШ показала значительно менее выраженный болевой синдром и отсутствие необходимости в дополнительном введении опиоидных анальгетиков.

The study deals with two mostly discussed techniques ofpostoperative analgesia for total knee joint arthroplasty. Surgeries were performed under subarachnoid anaesthesia with intravenous sedation. 9 patients of first group in received prolonged femoral nerve blockade as a component of multimodal analgesia. 8 patients of second group received epidural infusion of naropine. If basic technique of analgesia was not effective patients received trimeperidine 20 mg intramuscular. Patients of second group had less pain syndrome (in order to visual analogue scale) and did not need additional administration of opioids.
