Abstract

The decree of the government of Kaliningrad region № 284 of 06.2009 about inclusion of oblast oncologic dispensary into the structure of Kaliningrad Regional Clinical Hospital was implemented at the end of 2009. The data base of the Kaliningrad regional cancer register was used to analyze observable and corrected three-year survival rates of patients with malignant neoplasms of mammary glands on population level during two periods-from 15.11.2006 to 31.12.2009 (group “BEFORE”) and from 15.11.2009 to 31.11.2012 (group ”AFTER”). In the group AFTER the observable and corrected survival rates were statistically more reliable than in the group BEFORE (p<0.001). The advantage of observable and corrected survival rates in the group AFTER remained having regards to stage of tumor growth, modes of diagnosis verification, clinical group, type and strategy of treatment (at the level from p<0.05 to p<0.001 or as a tendency p<0.05). It is supposed that in 2010-2012 an increase in survival rates of patients with malignant neoplasms of mammary glands was caused by involvement of material, human and organizational resources of Kaliningrad region clinical hospital into oncologic service of the population.

Решение Правительства Калининградской области № 284 (от 06.2009 г.) о включении областного онкологического диспансера в состав Калининградской областной клинической больницы (КОКБ) было реализовано к концу 2009 г. На базе данных популяционного регистра рака в Калининградской области исследованы наблюдаемая (НВ) и скорректированная (СВ) трехлетняя выживаемость больных со злокачественными новообразованиями (ЗНО) молочной железы (ЗНМЖ) на популяционном уровне в течение двух периодов с 15.11.2006 по 31.12.2009 г. (группа "До") и с 15.11.2009 по 31.11.2012 г. (группа "После"). В группе "После" НВ и СВ были статистически достоверно выше, чем в группе "До" (p < 0,001). При учете стадии роста опухоли, способов верификации диагноза, клинической группы, вида и стратегии лечения преимущество в НВ и СВ пациентов в группе "После " сохранялось (на уровне от p < 0,05 до p < 0,001 или как тенденция p > 0,05). Предполагается, что рост выживаемости больных с ЗНМЖ в 2010-2012 гг. вызван вовлечением в онкологическое обслуживание населения области материальных, кадровых и организационных ресурсов КОКБ.
