Abstract

The results of the treatment of 72 patients with appendiceal infiltration are given, the diagnosis has been made only during the operation that was performed in case of acute appendicitis or peritonitis (find infiltration). The main feature of dense infiltration is the granulation tissue that makes it impossible to differentiate the organs and tissue involved in the inflammatory conglomeration, to part them with in a blunt way, because of bleeding in case of rapture or slash. 18 patients with dense infiltrations were underwent appendectomy. There were the following complications: 3 cases of pelvic abscess, 3 cases of the infiltration of the right iliac, 2 cases of the fistula of the blind gut combined with the infiltration of the right iliac. 3 patients had spread festering peritonitis that resulted in death. Appendectomy is a tactic mistake in case of dense infiltration.

Приведены результаты лечения 72 больных с аппендикулярным инфильтратом, диагностированном во время операции, предпринятой с диагнозом острый аппендицит или перитонит. Кардинальным признаком плотного инфильтрата является наличие грануляционной ткани, что проявляется невозможностью дифференцировать вовлеченные в воспалительный конгломерат органы и ткани, разделить их тупым путем, а также кровоточивостью при разрыве или рассечении. У 18 больных с плотным инфильтратом выполнены аппендэктомии. Встретились следующие осложнения: тазовый абсцесс у 3 больных, инфильтрат в правой подвздошной области у 3, свищ слепой кишки, сочетающийся с инфильтратом правой подвздошной области у 2. Развился разлитой перитонит у 3 больных, закончившийся летально. Аппендэктомия при плотном инфильтрате является тактической ошибкой.
