О возможных маркерах вариантов течения послеоперационного периода после эндоскопической холецистэктомии

Abstract

В работе приводятся данные о возможных условиях и причинах удлинения сроков пребывания пациентов в хирургическом стационаре после выполнения ургентной эндоскопической холецистэктомии при различных формах острого холецистита. Установлено, что сочетание ряда преморбидных факторов, конкретных индивидуальных проявлений обострения заболевания и особенностей проведения вмешательства коррелируют с развитием в послеоперационном периоде разнообразной неспецифичной клинической симптоматики, связанной с расстройствами адаптации. Предполагается, что вероятной причиной такого течения болезни являются скрыто протекающие затяжные расстройства системного гомеостатического регулирования, возникающие на фоне интраоперационного стресса, которые не диагностируются общепринятыми методами мониторинга динамики констант организма. Показано, что использование нейросетевого подхода позволяет выявить группу пациентов с наиболее высокой вероятностью послеоперационных адаптационных расстройств и наметить пути ее снижения за счет модуляции факторов, относящихся к методике проведения эндоскопической холецистектомии.

Possible conditions and reasons of more prolonged period of patient‘s hospitalization after urgent endoscopic cholecystectomy performed due to different acute cholecystitis types were presented in the article. It was revealed that different combinatios of premorbid factors, individual differences in acute disease сlinical picture, and intraoperative technical peculiarities correlate with postoperative dysorders resulting in various nonspecific clinical symptoms. It is assumed that longer recovery is determined by hidden protracted disorders of systemic homeostatic regulation against the background of intraoperative stress that had not been diagnosed by routine methods of body monitoring. The investigation results show that neural analysis allows to detect patients with the highest possible level of postoperative disorders and to plan the ways of their minimizing due to modulation of modifying factors relating to endoscopic cholecystectomy.
