Abstract

Проведен комплексный анализ целесообразности применения повторной трансуретральной резекции (ТУР) мочевого пузыря (МП) в рутинной практике лечения больных немышечно-инвазивным раком мочевого пузыря (НМИ РМП) на основании уточнения влияния повторной ТУР МП на время безрецидивной выживаемости и оценки прогностического значения данной методики. В проспективное исследование были включены 2 группы больных НМИ РМП (Т1N×M0 G1-3 ), сформированные в зависимости от объема проведенного им хирургического лечения (однократная и повторная ТУР МП) при медиане наблюдения 26 мес. Терапевтический эффект ТУР МП, проведенной через 4−6 нед после первичной, был отмечен только для пациентов групп низкого и среднего риска рецидивирования. Также были продемонстрированы возможности использования повторной ТУР для прогнозирования риска раннего рецидива и выбора оптимальной хирургической тактики лучения больных НМИ РМП, позволяющие рекомендовать ее введение в рутинную клиническую практику.

A comprehensive analysis was made to study whether it was expedient to use repeat transurethral resection (TUR) of the bladder to treat patients with non-muscle invasive bladder cancer (NMIBC) in routine practice, by clarifying the impact of repeat TUR of the bladder on relapsefree survival time and estimating the prognostic value of this procedure. A prospective study enrolled 2 groups of patients with NMIBC (Т1NxM0 G1 3) made up according to the scope of surgical treatment (single and repeat TUR of the bladder) with a median follow-up of 26 months. The therapeutic effect of TUR carried out 4-6 weeks after primary TUR was noted only in patients at low and moderate risk for recurrence. The possibilities of using repeat TUR are also shown to predict risk for early recurrence and to choose the optimal surgical policy for patients with NMIBC, which make it possible to recommend its incorporation into routine clinical practice.
