Офтальмологические проявления лейкозов

Abstract

Произведен обзор литературы, посвящённой офтальмологическим проявлениям лейкозов. Рассмотрены различные варианты поражения глаз при лейкозе на фоне сопутствующей анемии, гипоксии, повышенной вязкости крови, а также обусловленные сдавлением тканей конгломератами опухолевых клеток и/или наличием прямой лейкемической инфильтрации. Офтальмологические заболевания могут возникать у онкогематологических пациентов в связи с оппортунистическими инфекциями и на фоне воздействия таких методов лечения, как химиотерапия, радиотерапия и трансплантация костного мозга. Достоверно оценить частоту вовлечения глаза в лейкемический процесс сложно, так как многие пациенты на ранних стадиях заболевания не имеют никаких симптомов. Тем не менее прогностическая значимость оценки поражения органа зрения при лейкозе высока, что требует тесного сотрудничества онкогематологов с офтальмологами и соответствующей корректировки как местной, так и системной терапии.

This paper includes a literature review on the ophthalmic manifestations of leukemia. Various eye disorder types in leukemia with concurrent anemia, hypoxia, increased blood viscosity, as well as the compression of tissues by tumor cells conglomerates and/or presence of direct leukemic infiltration were considered. Ophthalmic diseases in patients with hematological malignancies may occur due to opportunistic infections and the impact of such treatments as chemotherapy, radiotherapy and bone marrow transplantation. Reliable estimation of eye involvement rates in leukemic process is difficult, as many patients are asymptomatic at early stages of the disease. However, the predictive value of visual impairment assessment in leukemia is high, which requires close cooperation between ophthalmologists and hematologists, and appropriate adjustment of both local and systemic therapies.
