Современные методы диагностики предрасположенности к злокачественной гипертермии

Abstract

Злокачественная гипертермия - редкое заболевание, угрожающее жизни, проявляющееся у генетически предрасположенных пациентов развитием острого массивного рабдомиолиза при воздействии веществ-триггеров. Предрасположенность к злокачественной гипертермии (ПЗГ) наследуется по аутосомно-доминантному типу с различной пенетрантностью. "Золотым стандартом" диагностики ПЗГ является галотан-кофеиновый контрактурный тест (ГККТ), который представляет моделирование in vitro с помощью биопсийного материала реакции поперечно-исчерченной мышечной ткани на препараты-триггеры и характеризуется 99% чувствительностью и 93,6 % специфичностью. Генетический анализ менее инвазивен, но чувствительность его существенно ниже. Описаны оба метода, освоение которых необходимо для создания в России возможностей современной диагностики ПЗГ, и обсуждаются сегодняшние представления о роли каждого из них.

Malignant hyperthermia is a well-known rare life-threatening autosomal-dominant pharmacogenetic disease. The article deals with a halothane-caffeine contracture test. The test is a model of muscle reaction to triggers in-vitro and it is the “golden standard” for malignant hyperthermia susceptibility (MHS) diagnosis. Genetic analysis is less invasive, but its sensitivity is significantly lower. The review discusses both the methods which are essential to be completely reproduced in Russia, and their role in modern approach to MHS diagnosis.
