Abstract

Исследованы 4 схемы анальгезии: 1) нефопам на фоне контролируемой пациентом анальгезии (КПА) тримеперидином; 2) кетопрофен (100 мг каждые 12 ч внутримышечно) на фоне КПА тримеперидином; 3) комбинация нефопама и кетопрофена по указанной выше схеме на фоне КПА тримеперидином; 4) КПА тримеперидином в виде монотерапии в раннем послеоперационном периоде у кардиохирургических больных. В каждую группу вошли по 20 кардиохирургичских больных (всего 80 пациентов) в возрасте от 40 до 70 лет. Введение нефопама и кетопрофена перед экстубацией больных снижает степень выраженности болевого синдрома в среднем на 90% и делает возможной раннюю активизацию пациентов. Сочетание нефопама и кетопрофена приводило к наиболее выраженному анальгетическому и опиоидсберегающему эффекту. Суммарная доза тримеперидина за 24 ч в среднем по этой группе достигала 14,7 мг на одного больного, что было в 4,9 раза меньше дозы, потребляемой в группе изолированной КПА тримеперидином. По динамике максимальной инспираторной емкости легких (МИЕЛ) получены наилучшие результаты в первых трех группах, что достоверно отличалось от группы изолированной КПА с 6-го часа от начала исследования. В целом нежелательные эффекты были связаны с введением тримеперидина и зависели от его дозы. В группе изолированной КПА частота тошноты, рвоты, головокружения, слабости была достоверно выше, чем в остальных группах.

4 combinations of analgesia were studied: 1) Nefopam and patient-controlled analgesia (PCA) with Trimeperidine; 2) Ketoprofen (100 μg each 12 hours intramuscular) and PCA with Trimeperidine; 3) Nefopam, Ketoprofen and PCA with Trimeperidine; 4) PCA with Trimeperidine as monotherapy in early postoperative period in cardio-surgical patients. 80 patients (age from 40 to 70) were divided into 4 groups, 20 patients in each group. Administration of Nefopam and Ketoprofen before extubation reduced the intensity of pain syndrome (in average on 90%) and promoted the early stirring up of patients. Combination of Nefopam and Ketoprofen provided the most expressed analgesic and opioids-saving effects. In this group average amount of Trimeperidine per 24 hours was 14.7 μg that was 4.9 times less than in group of PCA with Trimeperidine as monotherapy. Dynamics of maximal inspiratory capacity of the lungs in the first three groups was better than in group of PCA with Trimeperidine as monotherapy beginning from 6 th hour of study. In common undesirable effects was connected with Trimeperidine administration and depended on its dose. The frequency of nausea, vomit, dizziness and weakness was authentically higher in the group of PCA with Trimeperidine as monotherapy than in other groups.
