Мониторинг прогрессирования первичной открытоугольной глаукомы на фоне моно- и комбинированной гипотензивной терапии

Abstract

Цель работы оценить в сравнительном аспекте частоту прогрессирования первичной открытоугольной глаукомы (ПОУГ) на фоне длительной монои комбинированной местной гипотензивной терапии при диспансерном наблюдении больных. Материал и методы. Проведен ретроспективный анализ амбулаторных карт у 85 больных (165 глаз) ПОУГ I-III стадий с компенсированным в пределах среднестатистической нормы внутриглазного давления (ВГД), которые наблюдались в течение 5-7 лет. Из них 64 пациента (124 глаза) получали местное гипотензивное лечение в виде инстилляций только неселективного бета-адреноблокатора 0,5 % тимолола, а 21 больной (41 глаз) нефиксированную комбинированную терапию (в 20 глазах тимолол в сочетании с холиномиметиком пилокарпином и в 21 глазу тимолол с простагландинами, в основном, ксалатаном). Прогрессирование заболевания определяли по состоянию поля зрения (кинетической периметрии) и диска зрительного нерва (офтальмоскопии). Результаты. Анализ показал, что прогрессирование ПОУГ на фоне компенсированного в пределах среднестатистической нормы ВГД у больных, применявших неселективный бета-адреноблокатор тимолол наблюдалось в 1,3 раза чаще, чем у пациентов получавших нефиксированную комбинированную терапию блокаторы с простагландиновыми препаратами или холиномиметиками (62,1 % против 46,3 %). При этом дестабилизация заболевания при использовании комбинации тимолола с препаратами простагландинового ряда отмечалась почти в 2 раза реже, чем при сочетании его с пилокарпином (33,3 % против 60,0 %). Заключение. В качестве стартовой терапии первичной открытоугольной глаукомы необходимо значительно более широко и активно применять комбинированное лечение с использованием простагландиновых препаратов, что позволяет уменьшить частоту прогрессирования ПОУГ.

Purpose of the study is to assess comparatively the frequency of primary open angle glaucoma (POAG) progression along with prolonged mono and combined antihypertensive therapy at regular medical check-up of patients. Materials and Methods. It was performed a retrospective analysis of out-patient medical records in 85 patients (165 eyes) with POAG stage I-III compensated within average rate of intraocular pressure (IOP), which were followed for 5-7 years. Of these, 64 patients (124 eyes) received topical antihypertensive treatment in the form of instillations with only non-selective beta-blocker timolol 0.5 % and 21 patients (41 eyes) non-fixed combination therapy (20 eyes timolol in combination with cholinomimetic pilocarpine and in 21 eyes timolol with prostaglandins, mainly xalatan). Progression of the disease was defined by the state of vision fields (kinetic perimetry) and optic disc (ophthalmoscopy). Results. The analysis showed that the POAG progression against the IOP compensated within average rate in patients treated with non-selective beta-blocker timolol was observed in 1.3 times more frequently than in patients treated with non-fixed combination therapy β-blockers with prostaglandin drugs or cholinomimetics (62, 1 % vs. 46.3 %). Thus destabilization of the disease while using a combination of timolol with drugs of prostaglandin series was marked by almost 2 times less than at combination with pilocarpine (33.3 % vs. 60.0 %). Conclusions. The combination therapy using prostaglandin drugs that can reduce the incidence of POAG progression must be much more widely and actively used as the initial treatment of primary open angle glaucoma.
