Технические аспекты лапароскопической пластики грыж пищеводного отверстия диафрагмы: обзор литературы и собственный опыт

Abstract

В статье проведен обзор литературы и представлен собственный клинический опыт хирургического лечения 263 больных с грыжами пищеводного отверстия диафрагмы. Проанализированы технические особенности и сложности проведения лапароскопических антирефлюксных операций. Установлено, что возникновение интраи послеоперационных осложнений, как правило, связано с грубыми техническими ошибками, допускаемыми во время операции. Сделан вывод, что профилактика осложнений и рецидивов при выполнении лапароскопических пластик грыж пищеводного отверстия диафрагмы заключается в использовании современных высокотехнологичных способов диссекции и коагуляции, а также в имплантации легких композиционных протезов на ножки диафрагмы.

The paper presents literature review and own clinical experience of surgery in 263 patients with hiatal hernia. We analyzed technical specifics and difficulties of performance of laparoscopic antireflux operations. The research shows that intraand postoperational complications generally occur due to gross technical mistakes made during operation. We draw a conclusion that prevention of complications and recurrences at laparoscopic hiatal hernia repairs is based on usage of modern hich-tech methods of dissection and coagulation as well as on implantation of ligth composite prostheses on cruses of diaphragm.
