Анализ осложнений миниинвазивных технологий лечения пациентов с синдромом механической желтухи

Abstract

Experience of diagnostics and treatment of mechanical jaundice with application of miniinvasive surgical interventions for 74 patients is analysed. The structure of the complications meeting at miniinvasive technologies of treatment of patients a syndrome of mechanical jaundice is established: Gemobiliya (3 supervision), drainage migration (2 supervision), violation of passability of a drainage (3 supervision), zhelcheistecheniye in an abdominal cavity (1 supervision), suppuration of hypodermically fatty cellulose in the field of a drainage exit (1 supervision). On the basis of the received results conclusions are drawn that with mechanical jaundice with signs холангита it is necessary for patients within 24 hours from the moment of establishment of the diagnosis to execute adequate drainage of bilious channels in combination with complex intensive therapy. Modern low-invasive methods of a biliarny decompression, certainly, are a highly effective remedy for mechanical jaundice and холангита. But, as well as for any other surgical manipulations, for these methods complications, often heavy, difficult korrigiruyemy, not wearing, however, the fatal nature.

Проведен анализ диагностики и лечения механической желтухи с применением миниинвазивных хирургических вмешательств у 74 пациентов. Установлена структура осложнений, встречающихся при миниинвазивных технологиях лечения пациентов с синдромом механической желтухи: гемобилия (3 наблюдения), миграция дренажа (2 наблюдения), нарушение проходимости дренажа (3 наблюдения), желчеистечение в брюшную полость (1 наблюдение), нагноение подкожно жировой клетчатки в области выхода дренажа (1 наблюдение). На основании полученных результатов сделаны выводы, что пациентам с механической желтухой с признаками холангита необходимо в течение 24 часов с момента установления диагноза выполнять адекватное дренирование желчных протоков в сочетании с комплексной интенсивной терапией. Современные малоинвазивные методы билиарной декомпрессии, безусловно, являются высокоэффективным средством лечения механической желтухи и холангита. Но, как и для любых других хирургических манипуляций, для этих методов свойственны осложнения, не носящие, однако, фатального характера.
