Выбор способа родоразрешения пациенток с рубцом матки

Abstract

Основной задачей акушерства является снижение материнской и детской заболеваемости и смертности. Большая роль в решении этой задачи отводится своевременному и оптимальному родоразрешению. В современном акушерстве только наличие рубца на матке не может являться абсолютным показанием к повторному абдоминальному родоразрешению. Роды через естественные половые пути при тщательном отборе пациенток допустимы лишь при состоятельности рубца, удовлетворительном состоянии матери и плода в крупных специализированных центрах. При этом рекомендуется пристальное наблюдение как за роженицей, так и за плодом для своевременного выявления возможных осложнений и быстрого оказания высококвалифицированной помощи.

Reducing maternal and infant morbidity and mortality is the primary task of the obstetrics. Choosing the optimal delivery method and the right time for it is a core issue in performing this task. The presence of uterine scar solely can not be an absolute indication for a repeat cesarean section in modern obstetrics. Vaginal birth in thoroughly selected patients is only possible in large specialized centres, provided that the uterine scar is consistent and there is № doubt of the well-being of the mother and the fetus. Meanwhile, a thorough surveillance of both mother and the fetus for timely detection and urgent management of possible complications is recommended.
