Факторы риска возникновения синдрома фемороацетабулярного конфликта у детей и подростков при болезни Легга — Кальве — Пертеса

Abstract

Материалом исследования стали истории болезни и рентгенограммы тазобедренных суставов 294 пациентов с болезнью Легга — Кальве — Пертеса. Модифицирован индекс асферичности головки бедренной кости, который сочетает преимущества угла асферичности по Notzli (угловые измерения) и индекса Gosvig (нечувствительность к погрешностям измерения, связанным с ротацией бедра). Определено, что резидуальная деформация впадины вследствие болезни Легга — Кальве — Пертеса после окончания ее формирования (скелетная зрелость ≥ 32 балла) наблюдается в 87 % случаев. Установлено, что критериями прогнозирования возникновения синдрома фемороацетабулярного конфликта при болезни Легга — Кальве — Пертеса являются скелетная зрелость на момент начала заболевания, объем поражения и сумма имеющихся рентгенологических факторов риска. При скелетной зрелости ≥ 22 балла и объеме поражения по Catterall ≥ 3 тактика выжидания не только приводит к нарастанию вторичной деформации в стадии фрагментации, но и сокращает период ремоделяции.

The material of the study were medical records and radiographs of hip joints of 294 patients with Legg — Calve — Perthes disease. We modified index of femoral head asphericity, which combines the advantages of asphericity angle by Notzli (angular measurement) and Gosvig index (insensitivity to measurement errors associated with the rotation of the hip). It is determined that the residual deformation of cavity due to Legg — Calve — Perthes disease after its formation (skeletal maturity ≥ 32) was observed in 87 %. It is established that the criteria for predicting femoroacetabular impingement in Legg — Calve — Perthes disease is a skeletal maturity at the time of onset of the disease, the amount of damage and the amount of available radiological risk factors. At skeletal maturity ≥ 22 points and total destruction by Catterall ≥ 3 expectant management not only leads to an increase in the secondary deformation in the stage of fragmentation, but also reduces the period of remodeling.
