Острая патология грудного отдела аорты в практике терапевта

Abstract

Несмотря на более чем двухсотлетний багаж знаний об острой патологии грудного отдела аорты, большинство врачей общей практики не чувствуют себя уверенно в диагностике и лечении пациентов с данной группой заболеваний, характеризующихся высокой летальностью. Появление современных высокоточных методов диагностики патологии аорты способствует раннему выявлению данной группы заболеваний. Также стала возможным диагностика патологии аорты, имеющей сравнительно скрытое течение и не выявляемой ранее с помощью ангиографии и компьютерной томографии. Данный литературный обзор объединяет современные знания об этиологии, патофизиологии, симптоматике и лечебной стратегии трех наиболее изученных заболеваний, характерных для синдрома острой патологии грудного отдела аорты: расслоении аорты, интрамуральной гематоме и пенетрирующей язве стенки аорты.

Despite of more than 200 years of awareness and understanding of acute aortic disorders we continue to be hesitant with the diagnosis and management of these highly lethal diseases. Recent advances in imaging and therapeutic strategies have facilitated early diagnosis of acute aortic conditions. In addition, it has become possible to diagnose subtle forms of aortic disease and optimize our strategy accordingly. In this article, we summarized etiology, pathophysiology, clinical presentation and contemporary strategy in the treatment of the three most frequently recognized pathologies such as acute aortic disorder: aortic dissection, intramural hematoma and aortic ulcer.
