Abstract

Контроль эффективности реабилитационных мероприятий у больных артериальной гипертензией может включать в себя динамическое наблюдение за вариабельностью сердечного ритма и внутрисердечной гемодинамикой, отраже! нием которой является диастолическая функция, однако вопрос о взаимоотношениях реакций вегетативного баланса и функции наполнения желудочков недостаточно изучен. Цель исследования – оценка динамики показателей диастолической функции левого желудочка при различных эф! фектах реабилитации у больных артериальной гипертензией. Объект исследования. 50 больных артериальной гипертензией с улучшением уровня адаптации в ходе физической реабилитации (n = 24) и без его улучшения по данным повторной активной ортостатической пробы (n = 26). Методы. Эхокардиотрафия с исследованием диастолической функции и кардиоинтервалография с помощью диаг! ностической системы ORTO expert. Основные результаты. В ходе реабилитации отмечено существенное увеличение скорости распространения раннего митрального потока (СРМП) на 13,5 % (р = 0,009) и скорости раннего движения латеральной части кольца митраль! ного клапана е’ на 9 % (р = 0,0058) у больных с повышением уровня симпатической активности. Область применения. Кардиология и кардиореабилитация. Выводы. Курс санаторной реабилитации привел к улучшению диастолической функции левого желудочка у больных с ар! териальной гипертензией, что более отчетливо проявилось у больных с повышением уровня симпатической активности.

Rehabilitation effective control of hypertensive patients may include dynamic monitoring of heart rate variability and diasto! lic function. However, the relationship between the reactions of vegetative balance and function of left ventricular filling are poorly understood. Aim – the purpose of this study was to evaluate left ventricular diastolic function of the dynamics at various rehabilitation ef! fects in hypertensive patients. The object of study. 50 hypertensive patients with the improvement of adaptation after physical rehabilitation (n = 24) and without improvements in active orthostatic test data (n = 26). Methods. Echocardiography with the study of diastolic function and cardiointervalography using diagnostic system «ORTO expert». Main results. During the rehabilitation course was a significant increase of the mitral flow propagation velocity (Vp) by 13,5 % (p = 0,009) and mitral annular lateral movement velocity е' by 9 % (p = 0,0058) in patients with increased sympathetic activ! ity. Scope. Cardiology and cardio!rehabilitation. Conclusion. The rehabilitation course has led to an improvement in left ventricular diastolic function in patients with hyper! tension, which is more clearly evident from the increased level of sympathetic activity patients.
