Структура оперативных вмешательств при лечении диафизарных переломов длинных костей нижней конечности в 2008 г

Abstract

There were analyzed the results of a survey of the chief substitute orthopedist-traumatologists of MH of regional state administrations of 9 Ukrainian regions as well as the data of anonymous survey of 856 orthopedist-traumatologists of 9 regions of Ukraine. The questions concerned the determination of the structure of surgical interventions in the treatment of diapheseal fractures of the lower extremity bones. In the structure of surgical interventions for open femur and tibia bones diapheseal fractures the methods of transosseous osteosynthesis by the apparatus for external fixation prevail (respectively, 74.75 and 81.0 %). External osteosynthesis (54.33 %) and intramedullar ostesynthesis without blocking (22.68 %) are mainly used for the surgical treatment of femur closed diapheseal fractures in Ukraine. Transosseous osteosynthesis with pin external fixation device (33.86 ) and external osteosynthesis (32.58 %) are commonly used for the surgical treatment of simple diapheseal fractures of the shin bones. Modern minimally invasive technology of osteosynthesis in the treatment of diapheseal fractures of long bones of the lower limbs is insufficiently used.

Проанализированы результаты опроса главных внештатных ортопедов-травматологов МЗ облгосадминистраций 9 областей Украины, а также данные анонимного опроса 856 врачей ортопедов-травматологов из 9 областей Украины. Вопросы касались определения структуры оперативных вмешательств при лечении диафизарных переломов костей нижней конечности. В структуре оперативных вмешательств при открытых диафизарных переломах бедренной кости и костей голени превалируют методы чрескостного остеосинтеза аппаратами внешней фиксации (соответственно 74,75 и 81,0 % случаев). При оперативном лечении закрытых диафизарных переломов бедренной кости в Украине наиболее часто используется металлоостеосинтез накостной пластиной (54,33 %), интрамедуллярный остеосинтез без блокирования (22,68 %). При оперативном лечении закрытых диафизарных переломов костей голени наиболее часто используются чрескостный остеосинтез спицевыми аппаратами внешней фиксации (33,86 %), а также металлоостеосинтез накостной пластиной (32,58 %). Современные малоинвазивные технологии остеосинтеза при лечении диафизарных переломов длинных костей нижней конечности используются в недостаточном объеме.
