Практические подходы к вестибулярной реабилитации

Abstract

Вестибулярная реабилитация сравнительно простой и эффективный способ лечения заболеваний периферического и центрального отделов вестибулярного анализатора. Диагностика вестибулярных нарушений, оценка выраженности повреждения и динамика состояния больных с головокружением осуществляются с помощью клинических тестов, характеризующихся достаточно высокой информативностью. Среди таких тестов исследование спонтанного и установочного нистагма, проба Хальмаги, тест «встать и пройтись на время», тест «устойчивости на одной ноге» и т.д. Вестибулярная гимнастика складывается из упражнений на стабилизацию зрения и тренировку равновесия. Медикаментозная терапия способна ускорять вестибулярную компенсацию и повышать эффективность вестибулярной гимнастики. Представлены результаты наблюдательной программы, целью которой было определение оптимальной продолжительности вестибулярной реабилитации в сочетании с приемом танакана у пациентов с односторонним непрогрессирующим периферическим вестибулярным расстройством. Установлено, что оптимальная продолжительность лечения составляет не менее 2 мес.

Vestibular rehabilitation is a simple and effective method of treatment of patients with diseases ofperipheral and central parts of vestibular analyzer. The diagnosis of vestibular disorders, estimation of severance and dynamics of patients state is conducted with use of clinical tests with high level of significance. Among such tests there are investigation of spontaneous and end-position nistagmus, Halmagy test, test "to stand up and go for time", test "on one leg equilibration" and so on. Vestibular gymnastic is composed of exercises for visual stabilization and balance training. Medicine treatment is able to accelerate vestibular compensation and increase the results of vestibular gymnastic. The results of observation research the purpose of which was to estimate the optimal duration of vestibular rehabilitation combined with tanakan treatment in patients with unilateral nonprogressive peripheral vestibular disorder are presented. The optimal duration of treatment was established to be as little as two months.
