Abstract

The study including 26 patients with pancreatonecrosis (PN) was aimed at estimating the efficiency of programmed control sanitation laparoscopy (CSL) of the omental bursa in the postoperative period. The decrease of postoperative complications from 49,8% to 26,6% (p<0,05), fatal outcomes from 20,5% to 15,4% (p<0,05) in comparison with 44 control patients who underwent traditional open sanitation of the omental bursa was documented. It was proved by the method of cardiointervalography that laparoscopic sanitation decreases the severity of intraoperative lesions, prevents the overtension of the adaptative mechanism and suppression of reserve compensatory potential of the patients’ organism in comparison with open sanitative surgery.

На опыте лечения 26 больных с панкреонекрозом доказана эффективность программированной контрольно-санационной лапароскопии сальниковой сумки в послеоперационном периоде. Частота послеоперационных осложнений снизилась с 49,8 до 26,6% (p < 0,05), летальных исходов — с 20,5 до 15,4% (p < 0,05) по сравнению с показателями в контрольной группе больных (44 пациента), у которых производили традиционные открытые санации сальниковой сумки. Методом кардиоинтервалографии установлено, что лапароскопические способы санации по сравнению с открытым методом снижают степень травматичности вмешательств, предупреждают перенапряжение механизмов адаптации и угнетение резервно-компенсаторных возможностей организма больных.
