Использование безрамочной КТ-навигации и локального фибринолиза в хирургии нетравматических внутримозговых кровоизлияний глубинной локализации

Abstract

Проанализирован начальный опыт применения навигационной системы Stealth Station® TREON®Plus фирмы Medtroniс (США) и локального фибринолиза актилизе во время операций по поводу первичных нетравматических внутримозговых кровоизлияний глубинной локализации. С использованием предложенного методологического подхода оперированы 12 больных. Достаточно подробно описаны техника выполнения операции пункционной аспирации под контролем нейронавигации и протокол проведения локального фибринолиза. Пункционная аспирация и локальный фибринолиз являются эффективными малотравматичными методами хирургического лечения кровоизлияний в области скорлупы и таламуса. Метод безрамочной КТ-навигации позволяет успешно пунктировать глубинно расположенные кровоизлияния различной формы и размеров, минуя функционально значимые зоны мозга.

The initial experience of Stealth Station® TREON®Plus navigation system (Medtronic, USA) and local fibrinolysis with aktilyze during operation because of primary non-traumatic deep intracranial hemorrhages was analyzed. 12 patients were operated using thas method. Techniques of image-guided aspiration and local fibrinolysis according to protocol were described in details. Image-guided aspiration and local fibrinolysis are effective microinvasive methods for surgical treatment of hemorrhages in area of putamen and thalamus. Image-guidance allows reaching deep hemorrhages of different form and size while leaving intact functionally significant areas of the brain.
