Способ хирургической профилактики ранних послеоперационных осложнений при гастрэктомии

Abstract

The estimation of direct results of application of a way non-reflux entero-enteroanastomosis (the demand for the invention № 2010154844) is spent at gastrectomy at sick of a stomach cancer. The comparative analysis gastrectomy is spent at 72 stomachs sick by a cancer. Applied gastrectomy with imposing esophago-intestinal anastomosis in updating of Sigala. Patients have been divided into 2 groups depending on a way of imposing entero-enteroanastomosis at gastrectomy. Between groups it has not been revealed significant distinctions on a sex, age, character of an accompanying pathology. 1 group – 42 patients operated with a traditional method, entero-enteroanastomosis on Braun. 2 group – 30 patients operated in the offered way of imposing entero-enteroanastomosis at gastrectomy. The received results of formation non-reflux entero-enteroanastomosis studied by means of ultrasonic, endoscopic and radiological methods of research. Ultrasonic research at patients after gastrectomy allows to reveal at an early stage of infringement of evacuation from a resulting loop, to watch dynamics and to predict a current. Application of a way non-reflux entero-enteroanastomosis at gastrectomy at sick of a stomach cancer has allowed to lower risk of development of a syndrome of a resulting loop and quantity of early postoperative complications.

Проведена оценка непосредственных результатов применения способа арефлюксного энтеро-энтероанастомоза (заявка на изобретение № 2010154844) при гастрэктомии у больных раком желудка. Сравнительный анализ гастрэктомий проведен у 72 больных раком желудка. Применяли гастрэктомию с наложением муфтообразного пищеводно-кишечного анастомоза в модификации Сигала. Пациенты были разделены на 2 группы в зависимости от способа наложения энтеро-энтероанастомоза при гастрэктомии. Между группами не было выявлено значимых различий по полу, возрасту, характеру сопутствующей патологии. 1 группа – 42 пациента, оперированных традиционным методом, энтеро-энтероанастомоз по Braun. 2 группа – 30 пациентов, оперированных предложенным способом наложения энтеро-энтероанастомоза при гастрэктомии. Полученные результаты формирования арефлюк¬сного энтеро-энтероанастомоза изучали с помощью ультразвукового (УЗИ), эндоскопического и рентгенологического методов исследования. Ультразвуковое исследование у больных после гастрэктомии позволяет выявить на раннем этапе нарушения эвакуации из приводящей петли, следить за динамикой и прогнозировать течение. Применение способа арефлюксного энтеро-энтероанастомоза при гастрэктомии у больных раком желудка позволило снизить риск развития синдрома приводящей петли и количество ранних послеоперационных осложнений.
