Анапластический рак поджелудочной железы с остеокластоподобными гигантскими клетками: особенности течения, лечение и прогноз

Abstract

Objective. To investigate treatment results of pancreatic anaplastic carcinoma in osteoclast-like giant cells patients. Material and methods. The article presents two clinical cases of this type of carcinoma: clinical behavior, diagnostic and treatment issues, early and late treatment results. Results. A postoperative complication developed in one patient (deep vein thrombosis of lower limbs), no postoperative mortality was observed. Two radically operated patients are alive without evidence of disease progression with the time of observation 57 and 84 months. Conclusions. Anaplastic carcinoma with osteoclast-like giant cells is a rare variant of pancreatic tumor. The origin of this disease is not clearly understood. It was shown that the only radical method is surgical treatment. Favourable outcome can be achieved in case of radical surgery. Usefulness of adjuvant therapy is not known so far.

Цель. Оценить результаты лечения больных анапластическим раком поджелудочной железы с остеокластоподобными гигантскими клетками. Материал и методы. В работе представлены два клинических наблюдения больных с данной формой патологии. Описано течение, лечебно-диагностические подходы, непосредственные и отдаленные результаты. Результаты. В одном случае отмечено послеоперационное осложнение (тромбоз глубоких вен нижних конечностей). Два радикально оперированных больных в настоящее время живы, признаков прогрессирования заболевания нет, сроки наблюдения 57 и 84 мес. Заключение. Показано, что единственно возможным методом радикального лечения является оперативное вмешательство, при этом ожидается хороший прогноз. Вопрос о целесообразности адъювантной терапии может быть окончательно решен только по мере накопления клинического материала.
