Abstract

Выполнено исследование сывороточной концентрации высокочувствительного СРБ (hsCRP) у 61 женщины с преждевременным излитием околоплодных вод (ПИОВ) при доношенной беременности и у 49 рожениц со своевременным излитием вод. Отмечено достоверное повышение уровня hsCRP у женщин с ПИОВ при наличии хронических инфекционных заболеваний (6,50 ± 0,68 мг/л и 3,05 ± 0,71 мг/л, p < 0,01). У родильниц с воспалительными изменениями в последе среднее значение hsCRP в группе с ПИОВ составило 8,31 ± 1,02 мг/л, в группе со своевременным излитием - 5,20 ± 0,78 мг/л (p < 0,05), по мере увеличения распространенности воспалительного процесса концентрация hsCRP возрастала в обеих группах. Диагностический порог концентрации hsCRP посредствам ROC-анализа составил 7,2 мг/л. Повышение концентрации маркера выше диагностического порога может указывать на риск распространения инфекционно-воспалительного процесса и диктует выбор активной тактики родоразрешения у пациенток с преждевременным излитием околоплодных вод при доношенной беременности.

High sensitive CRP (hsCRP) was measured in 61 women with premature rupture of membranes at term (PROM) and 49 with normal rupture of membranes. It was seen increased level of hsCRP in women with chronic inflammatory diseases in PROM group (6,50 ± 0,68 mg/l and 3,05 ± 0,71 mg/l, p < 0,01). We found a great difference in two groups in the case of inflammatory changes in placenta (8,31 ± 1,02 mg/l and 5,20 ± 0,78 mg/l, p < 0,05), spreading of inflammation was accompanied with increasing hsCRP concentration in both groups. Diagnostic threshold of hsCRP made by ROC-analysis was 7,2 mg/l. Serum concentration of hsCRP more than 7,2 mg/l can show on the risks of spreading the inflammation in placenta and dictates to select active tactics in patients with premature rupture of membranes.
