Abstract

The purpose of studying characteristics of the course of pregnancy and delivery, as well as perinatal outcomes in women with Rh-conflict pregnancy, a retrospective analysis of 173 patients with histories of labor Rh-sensibilization in gestational age from 22 to 39 weeks hospitalized in Scientific Center of Obstetrics, Gynecology and Perinatology 2012-2014 years. In the forecast of severe hemolytic disease of the fetus play an important role "monotonic" or "jumpy" nature of the dynamics of Rh-antibody titer. The most informative method of diagnosing the severity of hemolytic disease of the fetus is the Doppler blood flow in the middle cerebral artery of the fetus, which allows to solve the issue of timely prenatal blood transfusions to the fetus. During pregnancy and delivery in women with Rh sensibilization was accompanied by a large number of complications (72.5% and 30.8%), there was a high incidence of preterm (39.5%) and operational (50.8%) births. In 148 neonates (85.6%) diagnosed with hemolytic disease of the newborn varying severity. 98 (72.0%) neonates with severe hemolytic disease of the newborn performed exchange transfusion

С целью изучения особенностей течения беременности и родов, а так же перинатальных исходов у пациенток с резус-конфликтной беременностью, проведен ретроспективный анализ 173 историй родов пациенток с Rh-сенсибилизацией в сроке гестации от 22 до 39 недель, госпитализированных в НЦАГиП в 2012-2014годы. Выявлено,что наиболее важным фактором риска развития резус иммунизации остается переливание крови без учета резус-принадлежности и отсутствие своевременной специфической профилактики путем введения антирезус-иммуноглобулина. В прогнозе тяжелых форм ГБП важная роль принадлежит «монотонному» или «скачущему» характеру динамики титра Rh-антител. Наиболее информативным методом диагностики тяжести ГБП является допплерометрия кровотока в СМА плода, позволяющая решить вопрос о своевременном ВПК плоду.Течение беременности и родов у пациенток с резус-сенсибилизацией сопровождалось большим числом осложнений (72,5% и 30,8%), отмечалась высокая частота преждевременных (39,5%) и оперативных (50,8%) родов. У 148 новорожденных (85,6%) диагностирована ГБН различной степени тяжести. 98 (72,0%) новорожденным с тяжелыми формами ГБН проведено ЗПК.
