Оптимизация хирургической тактики лечения острого калькулезного холецистита, осложненного холедохолитиазом, у больных с крайне высокой степенью операционно-анестезиологического риска

Abstract

Анализируется опыт оперативного лечения 88 больных с острым холециститом, осложненным холедохолитиазом, и крайне высокой степенью операционно-анестезиологического риска по шкале МНОАР (1989) за период с 1998 по 2009 г. С 1998 по 2003 г. наиболее часто применялось сочетание эндоскопической папиллосфинктеротомии и лапароскопической или «открытой» холецистэктомии. Широкое применение с 2004 г. лапароскопической и «открытой» холецистостомии с удалением конкрементов в сочетании с эндоскопической папиллосфинктеротомией позволило уменьшить летальность с 27,9 до 11,1% случаев.

For a 12-year period (1998-2009), 88 patients with extremely high operation risk were submitted to endoscopic, laparoscopic and «open» operations for acute cholecystitis, associated with bile duct stones. From 1998 to 2003 endoscopic sphincterotomy with laparoscopic and «open» cholecystectomy were the main procedures. Since 2004, when laparoscopic and «open» cholecystostomy with stones removal and endoscopic sphincterotomy with common bile duct exploration have become the main procedures, mortality rate decreased from 27,0 to 11,1%.
