Тактика ведения послеоперационного периода при острой тонкокишечной механической непроходимости

Abstract

Цель исследования: разработать и патогенетически обосновать оптимальную тактику ведения больных с острой тонкокишечной механической непроходимостью (ОТМН) в послеоперационном периоде. Материал и методы: проанализированы результаты лечения 68 (100%) больных с ОТМН, которым выполнена резекция нежизнеспособного участка кишки в зависимости от степени кишечной непроходимости. Выделены основная (I) группа – 40 (58,8%) больных, которым проводили отмывание тонкой кишки и энтеральное зондовое питание (ЭЗП) в раннем послеоперационном периоде, и контрольная (II) группа — 28 (41,2%) больных, которым динамическую кишечную непроходимость (ДКН) разрешали традиционными способами и осуществляли полное парентеральное питание до разрешения ДКН. Результаты: Сроки разрешения ДКН в I и II группах 30±9,8 и 46,3±13,85 ч соответственно; p<0,001. Послеоперационные осложнения в раннем послеоперационном периоде в I и II группах 2 (5,0%) и 9 (32,14%) соответственно; р=0,004. Летальность во II группе 2 (7,14%), в I группе летальных исходов не было. Сроки госпитализации в I и II группах 11,2±1,73 и 14,63±2,24 суток соответственно; р=0,004. Заключение: применение методики отмывания тонкой кишки и раннего энтерального питания в послеоперационном периоде позволяет избежать развития синдрома кишечной недостаточности, сократить количество послеоперационных осложнений и длительность пребывания в стационаре.

Objective: to develop and substantiate pathogenetic optimum management of patients with acute intestinal obstruction mechanical (AIOM) in the postoperative period. Material and Methods: The results of treatment of 68 (100%) patients with AIOM, who underwent resection of nonviable bowel area, depending on the degree of intestinal obstruction. Identify the main (I) group 40 (58.8%) patients who underwent intestinal laundering and enteral tube feeding (ETF) in the early postoperative period, and the control (II) group 28 (41,2%) patients who dynamic ileus (DI) allowed by conventional methods, and total parenteral nutrition was carried out to permit DI. Results: Dates permit DI groups I and II 30 ± 9,8 and 46,3 ± 13,85 hours, respectively; p <0,001. Postoperative complications in the early postoperative period in the groups I and II 2 (5.0%), and 9 (32.14%), respectively; p = 0.004. Mortality in group II 2 (7.14%) in group I had no deaths. Terms of hospitalization in groups I and II 11,2 ± 1,73 and 14,63 ± 2,24 days, respectively; p = 0.004. Conclusion: Application of the method of laundering of the small intestine and early enteral nutrition in the postoperative period to avoid the syndrome of intestinal failure, reduce the number of postoperative complications and length of hospital stay.
