Abstract

Исследованы распространенность полиморфизма генов TLR4 (С6898А) и TLR9 (A2848G) и их ассоциации с данными анамнеза и клинической картиной у 425 пациентов с прогрессирующей стадией вульгарного псориаза мужского и женского пола в возрасте от 18 до 80 лет. Установлены ассоциативные связи мутантного аллеля А гена TLR4 6898С/А с каплевидной формой псориаза, наличием псориаза у близких родственников больного, наличием сопутствующих инфекционных или паразитарных заболеваний, низкими показателями дерматологического индекса качества жизни, частым рецидивированием патологического процесса. Наличие мутантного аллеля G гена TLR9 (A2848G) ассоциировано со склонностью к экссудации псориатических элементов, снижением эффективности проводимой терапии, большим показателем PASI до начала лечения и меньшей его динамикой в период лечения.

The prevalence of TLR4 (C6898A) and TLR9 (A2848G) gene polymorphisms and their associations with case history and clinical picture were studied in 425 patients of both genders, aged 18-80 years, with advanced stage of psoriasis vulgaris. Associations of TLR4 6898C/A mutant allele A with the guttat-type psoriasis, presence of psoriasis in the patient’s close relatives, presence of concomitant infectious or parasitic diseases, low dermatological quality of life index, and frequent relapses of the pathological process were detected. The TLR9 (A2848G) mutant allele G was associated with liability to exudation of psoriatic elements, inefficiency of therapy, high PASI score before therapy and its lesser changes over the course of treatment.
