Профилактическая центральная лимфаденэктомия – безопасный метод выбора операции у больных раком щитовидной железы

Abstract

Представлены непосредственные и отдаленные (более 10 лет) результаты применения центральной лимфаденэктомии (ЦЛАЭ) (удаление 6-й группы шейных лимфатических узлов) у 912 больных первичным раком щитовидной железы (РЩЖ) в Санкт-Петербургском центре эндокринной хирургии и онкологии в период с 1973 по 2011 г. Установлено, что ЦЛАЭ под визуальным контролем возвратных гортанных нервов и околощитовидных желез должна выполняться одномоментно при всех первичных операциях по поводу дифференцированного РЩЖ. Убедительно показано, что прецизионная техника оперирования позволяет избежать тяжелых осложнений – травм возвратных нервов и гипопаратиреоза.

The paper gives the immediate and late (more than 10-year) results of central lymphadenectomy (CLAE) (removal of the 6 th group of cervical lymph nodes) in 912 patients with primary thyroid cancer (TC) in the Saint Petersburg Center for Endocrine Surgery and Oncology in 1973 to 2011. It is established that CLAE under the visual guidance of recurrent laryngeal nerves and parathyroids should be simultaneously performed during all primary operations for differentiated TC. It is clearly brought out that precision surgical techniques can prevent severe complications – recurrent nerve injuries and hypothyroidism.
