Новый подход к исследованию тимопоэза при тимомегалии у детей

Abstract

В статье представлены свидетельства ослабления эмиграции Т-клеток из тимуса в периферический отдел иммунной системы у детей с тимомегалией, т.е. ослабления у них процессов Т-лимфопоэза в тимусе. Оценку эмиграции Т-клеток из тимуса проводили двумя методами: полимеразной цепной реакции в реальном времени (исследовали уровень Т-рецепторных эксцизионных колец (T-cell receptor excision circles - TREC) и проточной цитофлюориметрии (исследовали содержание CD3 +CD4 +CD45RA +CD31 +-клеток). Показано, что при тимомегалии у детей до 10 лет статистически значимо снижено количество копий TREC в пересчете на 1000 лимфоцитов. Уровень TREC у детей с тимомегалией до 2-летнего возраста соответствует нормальным показателям, свойственным примерно 8-летним детям, у которых функциональная активность тимуса снижена по сравнению с таковой у детей более раннего возраста. Параллельно со снижением уровня TREC с возрастом у детей снижается процентное и абсолютное содержание в крови CD3 +CD4 +CD45RA +CD31 +-клеток. Однако снижение содержания Т-клеток, недавно эмигрировавших из тимуса (Recent thymic emigrants - RTE), с возрастом по фенотипическому маркеру CD31 не столь интенсивно, как снижение RTE по уровню TREC. Результаты оценки содержания CD3 +CD4 +CD45RA +CD31 +- клеток в крови у детей с тимомегалией в возрасте от 2 до 10 лет и детей без тимомегалии той же возрастной группы показали, что при тимомегалии количество клеток этой субпопуляции не изменяется.

The evidence of weakening of T cells emigration from thymus to peripheral immune system in children with thymomegaly is presented. It means the thymopoiesis is reduced in children with thymomegaly. The thymic emigration was investigated by two methods: real time PCR (the TREC level was measured) and flow cytometry (the number of CD3 +CD4 +CD45RA +CD31 + cells was measured). It was shown that the number of TREC copies per 1000 lymphocytes is significantly decreased in children with thymomegaly till 10 years. The TREC level in children with thymomegaly till 2 years approximately fits to the normal level of 8 years old children at which functional thymic activity is reduced in comparison with smaller children. In parallel with TREC level decrease with age the percentage and absolute content of CD3 +CD4 +CD45RA +CD31 + cells in blood is also decreased. However the TREC reduce is more intensive than CD3 +CD4 +CD45RA +CD31 + cells number decrease. The investigation of CD3 +CD4 +CD45RA +CD31 + cells number in blood of 2-10 years children with thymomegaly and without it showed that the number of cells of this subpopulation in thymomegaly is not changed.
