Abstract

Проведено исследование регуляторно-клеточных и гуморальных механизмов иммунного ответа в патогенезе травматической болезни, осложненной пневмонией, в зависимости от злоупотребления алкоголем в анамнезе. Обследованы 62 пациента с сочетанной черепно-мозговой и скелетной травмой, осложнённой нозокомиальной пневмонией, из них 30 человек с алкогольными проблемами. Возраст обследованных больных от 28 до 50 лет, мужского пола, в среднем 34 ± 1,6 года. Длительность развития нозокомиальной пневмонии составила в среднем 48,2 часа при колебаниях от 48 до 96 часов. Исследован иммунный статус на момент развития нозокомиальной пневмонии и в динамике проводимой терапии в зависимости от алкогольного отягощения. Показано, что иммунодефицитное состояние, характерное для лиц с сочетанной черепно-мозговой и скелетной травмой, отягощенной алкогольным потреблением, проявляется не только недостаточностью в гуморальном звене иммунной системы и выражается в значимом снижении уровней иммуноглобулинов, но и недостаточностью клеточного звена иммунитета.

A study of regulatory and cellular and humoral immune response mechanisms in the pathogenesis of combined craniocerebral and skeletal injury complicated by pneumonia, according to a history of alcohol problems. The study included 62 patients with combined craniocerebral and skeletal injury complicated by nosocomial pneumonia, including 30 people with alcohol problems. Age examinees of 28 to 50 years old, male, an average of 34 ± 1,6 years. Duration of nosocomial pneumonia averaged 48,2 hours with fluctuations from 48 to 96 hours. Investigated the immune status at the time of development of nosocomial pneumonia and in the dynamics of therapy, depending on the complication of alcohol. It is shown that immunodeficiency characteristic of individuals with combined cranial and skeletal trauma, burdened by alcohol consumption, failure is manifested not only in the humoral immune system, and is expressed in a significant decrease in the levels of immunoglobulins, but in the failure of cellular immunity.
