Abstract

Изучены показатели метаболизма железопорфиринового комплекса и функционального состояния печени у 155 больных с гемохроматозом. Дана объективная оценка их роли в своевременной и правильной постановке диагноза на ранних этапах развития патологического процесса. Порфириновый обмен при наследственном гемохроматозе (НГХ) характеризуется нарушенными и нестабильными показателями, затрагивающими все этапы синтеза гемоглобина. При НГХ с сопутствующей кожной порфирией изменения в содержании сывороточного железа и сывороточного ферритина ассоциируются с нарушенным синтезом аминолевулиновой кислоты, порфобилиногена и со степенью активности фермента порфобилиноген дезаминазы. Диагностическую ценность функционального состояния печени наряду с трансаминазами представляет исследование экскреции копропорфирина с мочой. Выявленные изменения в порфириновом обмене при гомозиготной форме НГХ носят постоянный и необратимый характер.

A hundred of patients with hemochromatosis have been tested to analyze ferrum metabolism key figures versus liver functional. An objective evaluation of their role in a timely and correct diagnosis at an early stage of pathological has been given. Porphyrin metabolism of the patients with hereditary hemochromatosis is characterized by very defective and unstable rates affecting all the stages of hemoglobin synthesis. For such patients with hereditary hemochromatosis with associated porphyria cutanea tarda the change in ferrum metabolism correlates with the defective aminolevulinic acid and porphobilinogen synthesis. The analysis of the coproporphyrin excreted in the urine has a diagnostic value for the determination of the liver functional status. The detected changes in the porphyrin metabolism along the homozygous hereditary hemochromatosis are permanent and often irreversible.
