Трансанальная эндоскопическая Микрохирургия - место в хирургическом лечении рака прямой кишки

Abstract

Вопрос о показаниях к локальному иссечению опухоли у больных раком прямой кишки 1 стадии до сих пор остается дискутабельным. В исследование было включено 80 больных раком прямой кишки. Основную группу больных составили 40 пациентов, которым была выполнена трансанальная эндоскопическая резекция прямой кишки. В группу сравнения были включены 40 больных, которым было проведено традиционное радикальное хирургическое лечение. Изучены ближайшие и отдаленные результаты лечения больных “ранним” раком прямой кишки. Выполнение трансанальной эндоскопической резекции прямой кишки сопровождается небольшим количеством послеоперационных осложнений (10%), позволяет пациенту сохранить высокий уровень качества жизни, экономически оправданно. Однако частота местного рецидива (10%) у больных “ранним” раком прямой кишки, которые могли бы быть излечены при выполнении трансанальной эндоскопической микрохирургии, является неоправданно высокой. Разработан алгоритм определения тактики лечения больного раком прямой кишки, являющего кандидатом для выполнения органосохраняющей операции.

The question of indications to local excision of tumor at patients with rectal cancer of 1 stage still remains discussable. Research included 80 patients with rectal cancer. The main group of patients was made by 40 patients to whom the transanal endoscopic resection of a rectum was executed. The group of comparison included 40 patients to whom traditional radical surgical treatment was carried out. The immediate and remote results of treatment of patients with "early" rectal cancer are studied. Performance of transanal endoscopic resection of rectum is followed by a small amount of postoperative complications (10%), allows the patient to keep a high level of life quality, economically reasonably. However the frequency of local recurrence (10%) at patients with "early" rectal cancer who could be cured when performing transanal endoscopic microsurgery, is unfairly high. The algorithm of definition the tactics of treatment the patient with rectal cancer showing the candidate for performance of organ-preserving operation is developed.
