Abstract

Выбор методов лечения пациентов с позвоночной спинномозговой травмой сводится к консервативному и хирургическому, в котором выделяют вмешательства с использованием переднего, заднего доступов и их комбинации. При наличии вентральной компрессии спинного мозга отломками разрушенных тел позвонков полноценная декомпрессия и восстановление передней колонны позвоночника достигается при переднем хирургическом доступе. Обобщен опыт лечения 9 пациентов с переломами торако-люмбального отдела позвоночника с использованием переднего доступа. Детально описывается хирургическая техника и обсуждаются преимущества использования переднего доступа.

Patients with spinal trauma may be treated with either nonoperative of surgical measures, which include anterior procedures, posterior procedures and combination of those. When anterior compression of neural structures is obvious, the anterior spinal surgery is the reliable method to decompress the cord adequately and to reconstruct and stabilize the anterior vertebral column. We summarize anterior spinal fusion surgical procedures performed in 9 patients with thoraco-lumbar injuries. Surgical techniques of the approaches used and those advantages are described in details.
