Гистопатоморфологические изменения кишечника при обтурационной опухолевой толстокишечной непроходимости

Abstract

Изучены особенности морфологических изменений в стенке ободочной и тонкой кишки у 26 больных с обтурационной опухолевой толстокишечной непроходимостью (ОТКН). Выявлена динамика в развитии морфологических изменений кишечной стенки в зависимости от степени компенсации ОТКН, которая заключалась в прогрессирующем отёке, нарушении кровообращения с внутрисосудистыми предтромботическими изменениями и острой воспалительной реакции. Подобные острые изменения в кишечной стенке происходили на фоне хронических морфологических изменений, особенно у больных пожилого и старческого возраста, которые могут способствовать нарушению всасывательной функции кишки. На основании полученных данных можно сделать вывод, что следует воздерживаться от выполнения первичных межкишечных соустий на высоте ОТКН, а выполнение радикальных операций с формированием межкишечных анастомозов следует осуществлять не ранее, чем через 3-4 недели после устранения кишечной непроходимости.

Particular morphological changes in the wall of colon and small intestine have been studied in 26 patients with obturation tumorous intestine obstruction (OTIO). The dynamics in the development of morphological changes of intestinal wall were detected according to the rate of compensation of the OTIO, which lied in progressing edema, blood circulation disturbance with vessels prethrombus changes and acute inflammatory reaction. Such acute changes in the intestine wall occurred against a background of chronic morphologic changes, especially by the patients of elderly and senile age, which can contribute to the disturbance of sucking intestine function. On the base of received data can be made a conclusion about inadmissibility of realization of primary enterostomies on the height of OTIO, and realization of radical surgeries with forming interintestinal anastomosis should be done not earlier than in 3-4 weeks after elimination of ileus.
