Коррекция дисфункции эндотелия у женщин группы риска развития гестационной гипертензии

Abstract

In clinical studies in 119 pregnant women aged between 17 and 27 years, who after 20 weeks gestation diagnosed with gestational hypertension (GAH). At dividing the patients into groups, we considered baseline BP level in terms of till 20 weeks of pregnancy. Depending on the method of treatment, each group was divided into two subgroups A and B. Subgroup A included pregnant with GAH, who in complex treatment received L-arginine, subgroup B included pregnant with GAH without correction. Studies have shown that the development of the GAG and the frequency of hypertensive complications such as preeclampsia (PE) is determined in impaired NO-system and the degree of endothelial dysfunction (ED), which stimulates the vasoconstrictor effect of ET-1, causes endothelial damage and increased in the systemic circulation amount of circulating endothelial cells (CEC). Therefore, an important regulator of DBP in women with GAH was endothelial dysfunction. The use of L-arginine prevented the imbalance in the NOsystem in pregnant women with GAH. This was manifested by reduction to control values in the blood of pregnant women subgroups, 1st and 2nd groups eNOS enzyme activity and, consequently, a positive development content of NO, inhibition of the activity of iNOS, ONO2-, ET-1, CEC and ultimately restores DBP. Applications Larginine prevents the development of ED in pregnant women at risk of GAH.

В клинических исследованиях у 119 беременных в возрасте от 17 до 27 лет, у которых после 20 недель гестации диагностирована гестацион-ная артериальная гипертензия (ГАГ). При разделении пациенток на группы учитывали исходный уровень АД в сроки до 20 недель беременности. Каждая группа в зависимости от метода лечения была разделена на 2 подгруппы А и Б: подгруппу А составили беременные с ГАГ, которые в комплексе лечения L-аргинин подгруппу Б беременные с ГАГ без коррекции. Исследования показали, что развитие ГАГ и частота гипертензивных осложнений такие как преэклапсия (ПЭ) определяются нарушениями в NO-системе и степенью дисфункции эндотелия (ДЭ), что стимулирует вазоконстрикторные эффекты ЭТ-1, вызывает повреждение эндотелия и увеличение в системном кровотоке количества циркулирующие эндотелиальные клетки (ЦЭК). Следовательно, важным регулятором уровня ДАД у беременных с ГАГ была ДЭ. Применение L-аргинина предотвращало нарушение баланса в NO-системе у беременных с ГАГ. Это проявлялось восстановлением до контрольных значений в крови беременных подгруппА, 1-й и 2-й групп активности фермента eNOS и, как следствие, позитивным сдвигом содержания NO, угнетением активности iNOS, ONO 2 -, ЭТ-1, ЦЭК и в конечном счете восстанавливается уровень ДАД. Применения L-аргинина предупреждает развитие ДЭ у беременных группы риска развития ГАГ.
