Факторы формирования динамической клинической непроходимости у недоношенных детей

Abstract

Цель исследования выявление особенностей антеи интранатального анамнеза и клинических проявлений динамической кишечной непроходимости у недоношенных. Материалы и методы. Обследовано 60 детей, находившихся в стационаре с подтвержденным диагнозом динамической кишечной непроходимости. Они были разделены на три группы в зависимости от показателей массы тела при рождении (недоношенные с низкой, очень низкой и экстремально низкой массой). Методы диагностики: клинико-анамнестический, стандартные лабораторные и инструментальные исследования. Результаты. Выявлено множество неблагоприятных факторов анамнеза у детей с динамической кишечной непроходимостью: хроническая антенатальная гипоксия различного генеза, патологическое течение родов, искусственное вскармливание с первых дней жизни. Достоверных различий по преобладанию того или иного фактора формирования патологии в зависимости от гестационного возраста и параметров физического развития при рождении не выявлено. Симптомы динамической кишечной непроходимости также одинаково часто встречались во всех группах: увеличение объема застойного содержимого в желудке, наличие желчи в желудочном аспирате, упорные срыгивания, в том числе с примесью желчи, вздутие живота, отсутствие перистальтики.

Aim of investigation is revealing of the anteand intranatal anamnesis peculiarities and clinical manifestations of dynamic ileus in premature children. Materials and methods. 60 children, being at hospital with a confirmed diagnosis of dynamic ileus, were examined. They were divided into 3 groups depending on the body mass values at birth (premature children with a low, very low and extremally low mass). The methods of diagnosis: clinicoanamnestic, standard laboratory and instrumental investigations. Results. A multitude of unfavorable anamnesis factors in children with dynamic ileus: a chronic antenatal hypoxia of different genesis, pathologic flow of labor, artificial feeding from the first days of life is revealed. No trustworthy differences on that or other pathology forming factor prevailing depending on gestation age and physical development parameters at birth were revealed. The symptoms of dynamic ileus were also frequently encountered in all the groups: an increase of the congestive contents volume in stomach, a bile presence in a gastric aspirate, persistent regurgitations including a bile mixture, inflation of abdomen, a lack of peristalsis.
